We request at least 24 hours’ notice for appointment cancellations or rescheduling. Late cancellations (less than 24 hours’ notice) or no-shows may be subject to a cancellation fee. Please contact our office as early as possible if you need to change your appointment.
Monthly memberships may be cancelled with written notice. Cancellations received before the next billing cycle will take effect at the end of the current paid period. No partial refunds are issued for unused days within an active billing period. Members wishing to pause or cancel their membership should contact our office directly.
Program deposits are non-refundable once services have commenced. Refund eligibility for program fees depends on the stage of program completion and will be evaluated on a case-by-case basis. Foundational Health Mapping fees are non-refundable once the consultation has been completed.
Deposits paid for aesthetic procedures are non-refundable if the appointment is cancelled with less than 48 hours’ notice. Deposits may be transferred to a rescheduled appointment with sufficient advance notice.
Copayments, deductibles, and non-covered services are due at the time of service. Refunds for overpayments will be issued within 30 days of determination.
