We chose the name NEST to embody our mission of nurturing health and independence. Through our work with frail, homebound adults, we saw the need to focus on early, preventive care that treats the whole person. By addressing social factors and providing a supportive environment, we guide patients toward healthier, more independent lives, promoting longevity and graceful thriving.

Dr. Goris-Hernandez is a double board-certified family physician & Obesity medicine with over a decade of experience in geriatrics and a special interest in obesity medicine and aesthetics. She holds a Doctor of Medicine degree from Pontificia Universidad Católica Madre y Maestra, Dominican Republic (1999-2006), and completed postdoctoral training at The Brooklyn Hospital Center/Cornell affiliated (2011-2014), where they served as Chief Resident and contributed to transforming the Family Medicine Outpatient Clinic into a Patient-Centered Medical Home. She is fluent in Spanish and English. They have been recognized with several honors, including the "Caring Heart Award" nomination, and have actively participated in clinical research, publications, and various medical presentations.

Dr. Goris also previously served as a Senior Executive Partner and then as Senior Medical Director at Chen Seniors Medical Centers.

Dr. Rafael Hernandez, originally from the Dominican Republic, is Board Certified In Internal medicine and specializes in Geriatric Medicine with extensive experience in value-based medicine and clinical leadership. He completed his residency at Woodhull Medical Center in NYC and a Geriatric fellowship at NYU Langone.

During his training, he developed a passion for leadership and quality care, serving as a CIR representative and co-founding the House Staff Safety Council. Recognized for his contributions, he was named Fellow of the Year at NYU.
Dr. Hernandez previously served as the National Director of Acute and Post-Acute Care at Chen Medical. He later founded Domus Care Solutions to provide high-quality, home-based care for seniors, emphasizing their psycho-social needs.
